    Luminescence is constantly growing and evolving, especially
    with the unique contributions of each person who takes part
    in co-creating the event.

    Please fill out this form and send it over ASAP.  I highly recommend
    you first fill it out on a word document, save your word document,
    and then paste the info over here... (sometimes the information
    doesn't transfer and I've had to ask performers to re-send at least
    1-2x before. Hopefully this doesn't happen with you.. but just in case!)
